- Access the Nurse Portal to Request a Name Change or an Address Change
- In the top right-hand corner, select Profile Name
- Select 'Request Name Change' or 'Request Address Change' dropdown
- Complete application
Submit legal documentation* supporting your name change to the North Carolina Board of Nursing

*Examples of supporting legal documentation include:
- Social Security Card
- Change of name certificate as issued by a court (Required for any first name change)
- Birth Certificate
- Marriage License
- Divorce document reflecting name change
- Passport
- Immigration documents reflecting name change
- Driver's License
- Voter Registration (for address only)
- Federal Income Tax Return (for address only)
Note: Do not submit the original document.
